Enrollment:

To enroll in the school based health center for medical services, you MUST have a signed parental consent form on file. These forms are available in the clinic or in the office. All consents must be signed by a parent and either witnessed by clinic staff or notarized. Notary services may be obtained in the NMB Sr High Principal's office.
